Please join Montefiore Health System Executive Henry Chung, M.D., Senior Medical Director, Montefiore Care Management Organization and Professor of Psychiatry, on Wednesday, April 11, at 2 p.m. ET for a special AHA members-only webinar presentation and discussion highlighting the use of interactive technology to enhance collaborative care for low-income, ethnically diverse patients, with depression and anxiety in primary care. 

Montefiore Health System added a new element to behavioral health care using Valera Health, a smartphone app and platform, supporting an integrated behavioral health care model that facilitates patient engagement using two-way messaging, appointment reminders, tailored educational content, and automated self-assessments. The self-assessments, combined with data gathered from patients’ smartphones, alert care managers to changes in a patient’s condition that suggest the need for clinical intervention and help improve care planning.

  • How the collaborative care model (CoCM) links behavioral health services to primary care for low income, ethnically diverse patients.
  • How technology can help promote the scalability and sustainability of CoCM.
