Where Hospitals Focus Population Health Efforts

Other organizations’ population health activities focus on individuals living in a specific geographic area or community (66 percent), those experiencing a certain disease or condition (57 percent) or individuals for whom the organization has a financial risk (46 percent), according to the 2015 American Hospital Association Population Health Survey performed in collaboration with the Public Health Institute.

The survey also revealed that 91 percent of organizations agree or strongly agree that population health is aligned with their organization’s mission, and 80 percent agree or strongly agree that population health is aligned with their hospital or health system’s clinical integration strategy.

However, only 56 percent of organizations agree or strongly agree that their hospital or health system engages in programs to address such socioeconomic determinants of health as poverty, housing and violence.
