Addressing Social Determinants of Health
Social determinants of health are the circumstances in which people are born, grow up, live, work and age, and the systems put into place to deal with illness. These circumstances have tremendous effect on one’s health. And, they can impact anyone, regardless of age, race, or ethnicity.

It also discusses hospitals and health systems’ role in addressing the social determinants of health, and offers additional resources and practical case examples.
